Referral Payments

To thank you for your referral we pay you 8% of the sale amount of equipment installed to that customer.

Vendor Application

The purpose of the vendor application is to get your business registered in our system. This registration will allow tracking for commission payments. Please fill out every section below to allow us to complete your registration.

All leads will become the sole property of Water Doctor of Washington Inc. The referral commission will be paid for every system sold, installed and where funds for installation have been collected, per the "Vendor Application." Commission payments are paid on or before the 10th of the following month proceeding the sale and system installation and funds have been collected. This agreement may be cancelled with 30 days written notice by either parties to the agreement.

Vendor Agreement

Commission to Retail Vendors*

Sale of equipment - 8% on sale amount before taxes installed within a calendar month.
Payment of compensation to Preferred Vendors will be made once each month on the 10th of the following month for previous installs along with a report of all referrals sent to Water Doctor of Washington, Inc. by the preferred vendor.

Sales

Within Water Doctor's trade area, all referrals from preferred vendors will be followed up within two business days from receipt. Arrangements will be made to do an on site water analysis and sale of the appropriate water treatment equipment by a Water Doctor Water Specialist.

Installations

All installations will be done by Water Doctor of Washington, Inc. an authorized Kinetico Dealer.

Inventory

Water Doctor of Washington will be responsible for maintaining inventory levels and all costs related to inventory.

Scheduling

Installation dates and times will be coordinated by Water Doctor of Washington personnel only.

IRS Form W-9

Form W-9 is a commonly used IRS form for providing necessary information to a person or company that will be making payments to another person or company. One of the most common situations is when someone works as an independent contractor for a business.

To finalize this application you will need to upload a copy of a W-9 along with your application. If you do not have a current W-9 please download a recent copy of the IRS form W-9 Here. Once you have that filled out attach it using the link at the bottom of the application.

Vendor Application

*Commission will be paid to a vendor for any referral that turns into a sale. Just fill out the Lead Registration Sheet and send it to us for proper recording and follow through. No other effort is required on your part, CASH for a lead that turns into a sale. Its that simple.
